I know that water polo can get pretty rough, as can other sports. Do I have to worry about my young nephew starting out? Any advice I can give him?

As long as your nephew is starting with other players of the same ability (and they're all beginners), violence will probably be at a minimum. At that point, they're all mostly concerned with keeping their heads above the water and figuring out where the ball is.
That being said, roughness is part of the game. He'll figure that out himself as his skills progress and he starts matching up with other, stronger players. A few things I would tell him:
1. Don't start it yourself, because dirtiness can escalate pretty quickly.
2. The dirtiest players are often the least skilled and rely on their illegal moves. If things are getting out of control, tell a ref or coach. A bruise is understandable, but blood and broken bones should be avoided.
3. Learn common grabs and tricks and how to escape them, and/or help the refs see the illegal behavior.
